Lilly’s Foundayo at 17.2 mg outperforms 25 mg oral semaglutide on weight loss and A1C reduction, new indirect treatment comparison finds

Foundayo at 17.2 mg was linked to 1.5 percent greater weight loss and 0.3 percent greater A1C reduction than 25 mg oral semaglutide at 52 weeks

These additional findings build on results from the ACHIEVE-3 head-to-head study, in which both the 9 mg and 17.2 mg doses of Foundayo produced greater A1C and weight reductions than 14 mg oral semaglutide

Eli Lilly and Company, maker of Zepbound, announced results from a new analysis showing that Foundayo (orforglipron) at 17.2 mg was associated with significantly greater weight loss and improved blood sugar control compared with 25 mg oral semaglutide in adults with type 2 diabetes. The findings come from an indirect treatment comparison between Foundayo 17.2 mg and oral semaglutide 25 mg, drawing on data from the ACHIEVE-3 and PIONEER PLUS studies. Presented at the 62nd Annual Meeting of the European Association for the Study of Diabetes in Milan, Italy, the results add to a growing body of evidence supporting Foundayo’s effectiveness.

Deborah Horn, D.O., director of the Center for Obesity Medicine at McGovern Medical School at UTHealth Houston, said that for someone managing type 2 diabetes, choosing between treatment options is a decision that affects daily life. She said having comparative data gives clinicians the ability to make informed treatment choices tailored to each patient’s individual needs.

Using an indirect treatment comparison based on data from ACHIEVE-3 and PIONEER PLUS, Foundayo at 17.2 mg showed significantly greater weight loss and A1C reduction than oral semaglutide at 25 mg over 52 weeks. On average, participants taking Foundayo lost 1.5 percent more body weight than those on oral semaglutide (95% CI: -2.7% to -0.2%). Foundayo also lowered A1C by an additional 0.3 percent compared with oral semaglutide (95% CI: -0.6% to -0.1%). The analysis was adjusted for gender, age, baseline A1C, and baseline weight. Additional analyses using different statistical approaches produced consistent findings, with Foundayo at 17.2 mg showing 1.5 to 2.4 percent greater weight loss and 0.3 to 0.6 percent greater A1C reduction compared with oral semaglutide at 25 mg.

Rachel Batterham, OBE, MBBS, Ph.D., FRCP, senior vice president of medical innovation and external engagement at Lilly Cardiometabolic Health, said head-to-head trials remain the gold standard for comparing two medicines directly, but when that data isn’t yet available, other analytical approaches can still offer useful insight. She said the results of this indirect comparison suggest Foundayo may deliver A1C and weight reduction comparable to the 25 mg dose of oral semaglutide. She added that these findings, combined with Foundayo’s straightforward dosing that doesn’t require fasting or water restrictions, position it as a potentially foundational therapy for adults managing type 2 diabetes worldwide.
